The best chance of conceiving happens during the days leading up to ovulation, and the day itself. Unfortunately, women with irregular cycles may not ovulate reliably and can find it hard to know for sure when they are ovulating. Induced ovulation involves medication to create a more certain ovulation period, giving you a better chance of conceiving.

Ovulation is the moment that an egg is released from your ovaries during your menstrual cycle. On average, this occurs 14 days before the start of your period, but it does depend on your period cycle length. The day of ovulation and the 5 days leading up to it are known as the ‘fertile window’ — the time during your menstrual cycle that you are most likely to get pregnant.

Many women may struggle to accurately track their ovulation though, which can impact on their ability to conceive. Ovulation induction may be helpful for individuals who:

  • have irregular, infrequent or absent menstrual cycles;
  • have unexplained infertility;
  • are not spontaneously ovulating; or
  • have a low sperm count or poor sperm motility.
 

Ovulation is monitored with ultrasounds and by examining blood samples, allowing us to get a firm idea of how the egg is developing and maturing. We use this information to pinpoint the right time for you to have intercourse or for us to use additional treatments to prompt fertilisation.

The ovulation induction process

Ovulation induction is a minimally invasive process, often used early in a fertility journey before moving on to more complex treatments. Our ovulation induction procedure looks like:

  1. Stimulation: Patients either use oral medication or at-home hormone injections to stimulate ovulation. For oral medication, patients will take an ovulation tablet from Day 2 or 3 of their menstrual cycle for 5 consecutive days. If ovulation injections are used, patients will use a hormone injection for 8 to 14 consecutive days, from Day 3 of their cycle. Our specialists will help you feel comfortable with the injection process and show you exactly how to administer each ovulation injection safely.
  2. Monitoring: Your cycle is closely monitored with ultrasounds and blood tests so that we can watch as the follicles in your ovaries mature. This helps us to identify your fertile window and the best time for you to try for a baby.
  3. Ovulation: Often the egg will release on its own and allow for fertilisation. However, if this doesn’t happen, we can use another injection to trigger ovulation. This injection mimics your natural hormonal surges during ovulation and is very safe.
  4. Fertilisation: During your fertile window, we recommend having intercourse regularly — at least every 2 days — to improve your chances of conceiving. In some situations, instead of intercourse you may use artificial insemination to directly inject sperm into the uterus which ensures that the sperm is placed closer to the egg, hopefully prompting fertilisation. Around 2 weeks after the final day of your fertile window, we will use a blood test to find out if you’re pregnant.
 

While we know it will feel disheartening if you learn that you haven’t fallen pregnant, we encourage you to stay upbeat. Getting pregnant, even through fertility treatments and with specialist help, often takes multiple tries. We’re always there to help in every part of your fertility journey.

Risks of ovulation induction

Due to the use of hormone stimulation medication, some women will experience exaggerated symptoms of their usual menstrual cycle, including breast and abdominal tenderness, bloating and nausea. This may develop further into Ovarian Hyperstimulation Syndrome (OHSS), but this is rare. We will help you manage any ovulation induction side effects and symptoms that arise.

Use of the medication also carries the risk of multiple births when more than one egg matures. Our monitoring can help us identify if this has happened, but if it does we would advise you against having intercourse, as this can lead to a multiple pregnancy (when more than one embryo is carried in the uterus at the same time). Carrying more than one fetus can increase the risk of complications in your pregnancy or prompt early labour, which can lead to more health problems for you and your baby.

Unfortunately, going through ovulation induction treatment or any other fertility treatment does not guarantee you will be able to get pregnant. We truly wish we could guarantee that for every patient, but we make every effort possible to give you the best chance of becoming pregnant and having a baby.

In many cases, our team can advise you on when would be the best time for intercourse to coincide with ovulation after it is induced, in a process called Timed Sexual Intercourse (TSI). However, in some cases it may be more beneficial to combine induction with Intrauterine Insemination (IUI), where we can place sperm directly into the uterus and improve the likelihood of fertilisation. Every couple and individual is unique and your fertility journey will depend on your personal circumstances.
