What are uterine fibroids?

Fibroids (also known as uterine myomas) are benign tumours that occur in the uterine muscle. They are common in women of reproductive age; in fact, up to 80% of women experience them at some point. This means we see them often at Create Fertility.

Fibroids vary in size and number. Some are so small that they cannot be seen with the naked eye, while others have been known to grow to the size of a grapefruit or even larger. If they reach this size, they may distort the inside and/or outside of the uterus (leading to an enlarged or irregular uterus).

It is important to note that fibroids are not cancer. They very rarely turn into cancer, and they have not been linked with a higher risk of uterine cancers either. They may, however, cause severe pain and abnormal bleeding, pregnancy complications, and affect fertility.

While many people with fibroids don’t experience any symptoms, others do. Some of the more common symptoms include:

  • Heavy and painful periods
  • Longer periods
  • Bleeding between periods
  • Pelvic pain or pressure symptoms
  • Frequent urination
  • Painful sexual intercourse
  • Constipation

If left untreated, fibroid symptoms usually improve after menopause.

Do fibroids affect fertility?

While not everyone with fibroids will experience fertility issues, the size and location of the growths have been known to affect it. This may be due to several factors:

Submucosal fibroids (which grow on the inner wall of the muscle layer) that are quite prominent or on a stalk may irritate the uterine lining. This may affect an embryo’s ability to implant.

Fibroids growing near the cervix may compress the vaginal canal, reducing the number of sperm that can pass through.

Submucosal and intramural fibroids (which grow within the muscle wall) may distort the shape of the uterus. This may decrease the area available for implantation, as well as affect the ‘micro-contractions’ that are used to transport sperm.

Fibroids may block or impair the fallopian tubes, preventing sperm from reaching an egg and/or the embryo from reaching the uterus.

Larger and submucosal fibroids that push into the uterine cavity may interfere with implantation and the placenta’s ability to attach properly.

Some fibroids appear to produce proteins that may affect the placenta’s ability to continue growing, which may lead to miscarriage.

Unfortunately, the exact cause of these growths is unknown. However, many factors are believed to play a role in their development, including:

  • Gene changes
  • Hormones (specifically estrogen and progesterone)
  • Extracellular matrix (which makes cells stick together)
  • Other growth factors (like insulin)

There are a few different types of fibroids based on where they occur in the uterus:

  • Subserous: Grows on the outer wall of the uterus
  • Intramural: Grows within the uterus’s muscle wall
  • Submucosal: Grows on the inner wall of the uterus

There are also pedunculated fibroids, which occur when any of these growths form a stalk.

After both endometrial ablation and hysterectomy, you will no longer be able to conceive or carry a pregnancy. If you would like to retain the ability to fall pregnant, your fertility specialist will likely recommend one of the other treatment options available.
